The elections department has appointed a 5 member team to probe the Puthalam, St Andrews ballot paper incident.
Deputy election commissioner M M Mohammed stated that this committee was appointed yesterday.
Following preliminary investigations if any official is found guilty steps will be taken to issue a charge sheet.
Meanwhile, addressing a media briefing held in Puththalam former provincial minister Ashoka Wadiga-magawa stated that Puthalam district secretary must take the responsibility.
He also stated that there was a slight mishap on his part and that he apologized.
However, Puthalam district secretary addressing a media briefing in Colombo stated that he will file action demanding 25 million rupees as compensation.
Former provincial Minister Ashoka Wadiga-mangawa stated that he is not in a position to pay any compensation.
